For subcontractor listing and substitution violations against the TSP cross-defendants for breach of Contract B221, B231 and B211 and for false claims, false records and false statement penalties, plus unfair and/or unlawful business practice penalties related to this conduct in the sum of $__________.


For Disadvantaged Business Enterprise violations against the TSP cross-defendants for beach of Contract B221, B231 and B211 and for false claims, false records and false statement penalties, plus unfair and/or unlawful business practice penalties related to this conduct in the sum of $__________.


For subcontractor licensing violations against the TSP cross-defendants for beach of Contract B221, B231 and B211 and for false claims, false records and false statement penalties, plus unfair and/or unlawful business practice penalties related to this conduct in the sum of $__________.


For quality assurance/quality control violations against TSP cross-defendants beach of Contract B221 and for false claims, false records and false statement penalties, plus unfair and/or unlawful business practice penalties related to this conduct in the sum of $__________.


For Buy American violations against the TSP cross-defendants for breach of Contract B221 and B231 and for false claims, false records and false statement penalties, plus unfair and/or unlawful business practice penalties related to this conduct in the sum of $__________.


For breach of contract, including actual and trebled damages, plus penalties related to the following submission of false claims/false records/false statements, unfair and/or unlawful business practices pertaining to Contract B221:

For liquidated damages in breach of Contract B221 against the TSP cross-defendants in the sum of $__________.


As against the Aetna Casualty & Surety Company, Fidelity & Deposit Company of Maryland and North American Reinsurance Corporation bonding companies, we find liability for the same TSP breaches of Contract B221 with respect to each of the foregoing matters in the sum of $__________.


We, the jury in the above-entitled action, also find that the unfair competition and/or unlawful business practices of the TSP cross-defendants separately warrant restitution and/or disgorgement of funds received from the MTA in the sum of $__________.
